At the bottom of your health record. Your whole history on one page, in the order a doctor reads it: allergies, who can decide for you, everything you take, your conditions, surgeries and vaccines. Medicines and supplements come as one list, because a supplement can clash with a medicine and a separate list is how one gets missed.
