Immunizations

There is no national vaccination register in this country, so this list is yours. Not sure is a real answer and we record it as one. For a vaccine that comes as a course, like shingles at two doses, we ask how many you have had and say Series not complete if there are more to go. Leave it blank if you do not know. Where you got it is what lets a pharmacy find the record again later. We never ask for a lot number.
